What is self-referral
Self-referral lets you sign yourself up to a health service directly, without speaking to your GP. Specific services in our local area are available in this way, which make it easier for you to access the support you need.
What services are available to self-refer?
Social Care
Are you concerned that you or a loved one needs extra help around the home, or perhaps equipment to help them manage? Your local council can assess your needs, find out more about Hammersmith & Fulham social services here.
Talking Therapies (Adults)
Psychological therapy can be very helpful to manage common problems like anxiety, stress or depression. You can find out more about self-referral here. Please note this is not a crisis support service, if you are worried about your own safety or the safety of a loved one, please do not hesitate to seek medical advice and click here for more urgent support.
Dementia and Memory Support
The Alzheimer’s society, our local dementia support service, are here to give personalised help and advice to any person, family member, carer or friend concerned about memory problems or dementia. Contact them here.
Physiotherapy
For an initial assessment, we now have a physiotherapist working directly at the practice. Nikhil can assess your muscular or joint problem and advise on how best to manage it, and if need be, refer you on for further tests.
Our local musculoskeletal (MSK) service is also available for direct referral from patients to see an NHS physiotherapist who will need longer term input. More information is available on their website.
Stopping Smoking
Stop Smoking London can help you on your journey to quit. The NHS website also has some straight-forward explanations about the health benefits of stopping smoking.
Drugs or Alcohol Service
The Drug and Alcohol Wellbeing Service (DAWS) is a place where you can get real and practical help to manage problematic use of drugs and alcohol. This service is run by an organisation called Turning Point. You can refer yourself and find out more by calling 0330 303 8080 or going here
Sexual Health
Sexual health services are free to use. Sexual Health London provides free and easy to use at-home tests. If you think you may have symptoms of a sexually transmitted infection then you can attend our local NHS sexual health clinics, 10 Hammersmith Broadway or the Jefferiss Wing, St Mary’s Hospital.
Maternity Care
Pregnant women can self-refer to NHS maternity services. Our local services include Imperial College (St Mary’s, Queen Charlotte’s) and Chelsea & Westminster.
